If you have lost power and now have food spoilage in your refrigerator or freezer use these forms to help make your claim reporting process a smooth one. Complete the food spoilage list for each appliance and return to Claims along with the Food Spoilage Cover Sheet and the Fraud Warning Disclosure form.
